Good Faith Estimate Notice
You Have the Right to Receive a “Good Faith Estimate” Explaining How Much Your Care Will Cost
Under the law, health care providers are required to give clients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including mental health services.
At REALIGN YOUR MIND COUNSELING SERVICES, LLC, we are committed to transparency and helping you make informed decisions about your care. If you are paying out of pocket for therapy (not using insurance), you have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency services.
What You Can Expect
-
You will receive a written Good Faith Estimate before your first scheduled session or upon request.
-
The estimate will include the cost of the initial intake appointment and ongoing therapy sessions, based on the frequency and duration you and your therapist discuss.
-
This estimate is not a contract. You are not obligated to receive all services listed in the estimate.
Your Rights
-
You have the right to request a Good Faith Estimate before scheduling a service.
-
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
